Social Services
The Social Services Department offers the following services to our residents, families and partners at NHC.
Education
- Educate residents, families and partners about cultural differences
- To keep up to date on medicare and other managed care insurance
- Educate residents,families and partners about advanced directives
Clinical
- Participate and provide therapeutic intervention to assist resident's in coping with their transition and adjustment to their stay at NHC
- Conduct ongoing psychosocial interviews on each resident
- Provide as needed counseling to residents and their families
Advocacy
- Promote and protect residents rights
- Promote the dignity and worth of each resident
- Build upon each resident's strengths and self esteem
Discharge Planning
- Coordinate each resident's discharge planning process
- Collaborate with NHC inter-disciplinary team members to develop individual patient discharge plans
